Please use this form to report a new claim to the OFPA. You should fill the form out completely, giving us as much detail as you can. Email the completed form to the OFPA at info@orfairplan.com.

The OFPA uses Independent local adjusters to investigate and adjust all our claims. As soon as we receive notice of a new claim, we immediately contact an adjuster, who will normally make a contact within 24 hours or sooner.
